To get started, please complete the Puerto Rico EOI Station Enrollment Package. You can choose to fill out the form in English or Spanish—simply select your preferred language below.

Once you make your selection, a secure window will open with the form hosted via Adobe Sign. Please take your time to read the document carefully and complete all required fields. You’ll need to have the following information ready:

• Your EOI business details
• Bank account information for monthly billing of equipment and services

After you complete and submit the form, a secure copy will be automatically sent to Worldwide for processing. A Worldwide Customer Service Representative will then reach out to the primary contact listed in your submission to:

• Provide deposit submission instructions for your financial institution
• Finalize and confirm your order
